What to Expect
When you come in for your first visit, we will ask you to fill out a New Patient Form, as well as a Medical Form. You can also choose to fill out our New Patient form online prior to your appointment.
*IMPORTANT: Any form submitted by email must be encrypted, to ensure it is secure. By submitting an unencrypted email form, you are consenting to an unsecured email. Alternatively, please print off the form, complete it, and bring it to our office.
During the appointment, our staff will get to know you and review your past medical and dental histories in order to provide you with the proper dental care. We will complete a full oral exam in order to help determine your dental needs.
Getting to Know You
We will take the time to learn more about your dental health and the goals you have. Understanding what you're looking for will help us develop and create a personalized plan to help meet these needs and expectations.
How long will my first appointment take?
Your first appointment will take approximately 60 minutes. This will allow us to conduct a thorough review, and create a plan to meet your dental needs.
What will happen during my first visit?
During your first appointment, we will:
- Review your oral care history, including any issues you have regarding your dental health.
- Perform a complete oral exam including x-rays to assess anything that cannot be seen with the naked eye
- Create a comprehensive and personalized dental health plan
